Student Code of Conduct Violation

This form is used to report a possible violation of the Student Code of Conduct and other behavioral concerns.

Any member of the College community (faculty, staff, or student) or any person who is unaffiliated with the College and who has knowledge of a possible violation of the Student Code of Conduct may file a complaint. Disruptive Student Removal

This form is used by faculty or staff to remove a disruptive student from the classroom or academic environment for a period of time. Questions? Contact the Office of Behavioral Intervention and Student Conduct (BISC) at 918-595-7150.

Academic Integrity Violation

This form is used by faculty to report major and moderate offenses that violate 91影视's Academic Integrity Policy. Minor offenses can be reported at the discretion of the faculty member. This form should be completed after the instructor's meeting with the student regarding the violation.

Customer Service Concern

This form is used to report a concern related to 91影视 customer service. Any member of the College community (faculty, staff, or student) or any person who is unaffiliated with the College may report a concern.

Classroom Concern

This form is used to report a concern related to a faculty member's administration of the classroom (generally used by students in their class). Direct resolution is encouraged when possible.

Employee Concern

This form is used to report a concern related to a 91影视 employee. Direct resolution is encouraged when possible.

Ethical Concern

This form is used for individuals to report what they believe, in good faith, to be ethical breaches, including fraud, theft, conflicts of interest that may violate the , or waste at the College.

Title IX Report Form

This form is to be used for reporting information necessary to initiate an investigation of alleged acts of dating violence, domestic violence, sexual harassment, sexual misconduct, or stalking. A report will prompt the Title IX Coordinator to: notify the Complainant about supportive measures and discuss the process to file a formal complaint, but it does not prompt a Title IX investigation. A formal complaint is when a complainant files a signed document alleging sexual harassment and requests the College to investigate the allegation.

Discrimination and Harassment Report Form

This form is to be used for reporting information necessary to initiate an investigation of alleged discrimination based on: race, color, national origin, sex, age, religion, qualified disability, status as a veteran, sexual orientation, gender identity, genetic information, or any other basis protected by applicable law. This form is also used to report alleged discrimination against pregnant and parenting individuals, gender expression, or retaliation.

The Americans with Disabilities Act (ADA) or Section 504/508 of the Rehabilitation Act of 1973 - Report a Concern

Please use this form to report any ADA/504/508 barriers you encounter at 91影视. Once you submit this form, the ADA/504 Coordinator will investigate and respond in writing within 15 business days of submission of the complaint. We do our best to address these in a timely fashion unless it is something that requires further action and/or expense. In that case, we will let you know whether this is something that is required under ADA/504/508 and, if so, our plan to make the improvements.

Course Final Grade Appeal

This form is used to file a course final grade appeal concerning a final course grade. If you have a concern about a grade assigned during a semester, please use the .

An attempt to resolve a course grade dispute with the course faculty member must be made prior to filing a Course Final Grade Appeal. The course final grade appeal can be filed no later than the last day of the following semester.

Appeal of Academic Integrity Violation Report

This form is used to file an Appeal of Academic Integrity Violation Report. Students may appeal the alleged violations and/or sanctions applied. The appeal must be filed by the student within five (5) business days following the meeting between the instructor and student where the violations and sanctions are discussed.

Petition for Refund of Tuition and Fees for Students with Hardship Circumstances

This report allows students to request a tuition refund or waiver due to a hardship. The student must not currently be enrolled in the class(es) in which the refund is being requested. Requests must be made no later than one year following the end of the semester in which the student was enrolled in the class(es).

This form is used to report any safety concerns or injuries that have occurred while on campus. To access this form, you will need to use your Student ID (T) number and provide your contact information. If this is an emergency or high-priority item, please contact Campus Police at 918-595-8888.
